How to use patient treatment sheet


Heads Up!

    Ensure the correct administration status is selected for each scheduled dose to maintain accurate clinical and audit records.

  • Always document the reason for modifying a prescription to maintain proper clinical traceability and audit compliance.

    Ensure the medication is intended to be discontinued before confirming the action. It can be resume if affirmed that it can be resumed.

  1. Log in to the system as a user in the Consultation or Nursing Department.

  2. On the homepage, open the patient’s folder.

  3. From the left menu, select In-Patient/Ward if the patient is admitted, or General/Out-Patient if not.

  4. Click on Treatment Report.

  5. On the page, locate the medicine that needs updating.

Administering Medication

  1. On the Treatment Sheet, click the relevant date cell for the medication and scheduled time.

  2. The Select Action modal will appear with the following options:

  3. Given — Record that the medication was administered to the patient.

    After selecting Given, enter the dosage given and click on save.

  4. Not Available — Indicate that the patient was unavailable at the time of administration.

  5. Yet to Give — Mark that the medication has not yet been administered.

  6. No Access — Indicate that the medication could not be administered due to access limitations.

  7. Refused — Record that the patient refused the medication.

  8. Remove — Clear or remove the previously recorded status.


  9. Select the appropriate option to update the treatment record.


Editing a Prescription

  1. On the Treatment Sheet, click the Edit button next to the medication.

  2. The Edit Prescription modal will open.

  3. Modify the required fields as necessary:

  4. Dosage — Update the prescribed quantity.

  5. Frequency — Adjust how often the medication should be taken.

  6. Number of Days — Specify the updated duration of treatment.

  7. Route — Select the appropriate administration route from the dropdown list.

  8. In the Note/Reason field, enter an explanation for the prescription change.

  9. Click Change to apply the updates.

Stopping a Medication

  1. On the Treatment Sheet page, locate the medication you want to discontinue.

  2. Click the Stop Medication button associated with the medication entry.

  3. Confirm the action when prompted.

Once confirmed, the medication will be discontinued and will no longer appear as an active item on the treatment schedule.
